This document is specific to known issues related to using your voice to dictate report content that results into misrecognized words or phrases, initiating a voice command that fails to work correctly, and/or the formatting of a recognized word or phrase is presented inaccurately. There are two tables including; a "Speech Recognition - Known Issues" table that provides a list of recognition issues related to misrecognized words or phrases, and a "Voice Command / Formatting - Known Issues" table that provides a list of voice command issues and formatting issues. The issues presented in each list are in no particular order and these lists are updated frequently to include workarounds and solutions, when available. If you are experiencing any of the known issues found in either of these lists, please revisit this solution periodically to check for changes in the status of the issue(s).

Speech Recognition - KNOWN ISSUES

Misrecognition of the phrases "intra and extra" or "intra or extra"

When dictating phrases in the PowerScribe 360 | Reporting client application that include the word "intra," the system sometimes places the following word right next to the word "intra" without a space between them. For example; when dictating the phrase, "intra and extrahepatic" the result appears as "intraand extrahepatic." Although "extrahepatic" is a word, "intraand" is not and will need to be corrected.

There is a Dragon patch (DNS 12.50.150.005) available to upgrade PS360R systems running versions 1.5, 2.0 and 2.5. This patch is already rolled into PS360R 2.1 systems.

WORKAROUND:

Perform the following to access the vocabulary editor to make a few changes that will allow the word "intra" to work more efficiently.

  1. Launch the PowerScribe 360 | Reporting client application and Login.
  2. Say Edit Vocabulary or click Speech > Vocabulary Editor to open the Vocabulary Editor.
  3. Say or type intra into the Written Form text box.
  4. In the result list, click the word intra- to highlight it.
  5. Click the Delete button to remove from the vocabulary.
  6. In the result list, click the second instance of the word intra-, and then click the Delete button to remove it.
  7. In the Written Form text box, type the word intra (in lower case) and click the Add button.
  8. Click the Close button to exit the Vocabulary Editor.

 

Misrecognition of the phrase "AC Joint"

When dictating the phrase "AC Joint" the recognition engine interprets "AC" as "BEFORE MEALS" as though it is part of a prescription dosage.

There is a Dragon patch (DNS 12.50.150.005) available to upgrade PS360R systems running versions 1.5, 2.0 and 2.5. This patch is already rolled into PS360R 2.1 systems.

WORKAROUND:

Perform the following to access the Auto-Correct feature and add an entry to change "before meals" to "AC":

  1. Launch the PowerScribe 360 | Reporting client application and Login.
  2. On the Menu bar, click Tools > Preferences.
  3. In the Preferences dialog, click AutoCorrect.
  4. In the Replace box on the left, type before meals.
  5. In the With box on the right, type AC
  6. Check the box labeled IC (Ignore Case).
  7. Click the Add button.

IMPORTANT: The option "Replace text during dictation" must be enabled to see the replacement text during the dictation of the report. if unchecked, the replacement occurs when the document is closed and you will not see this happen.

TIP: If you are dictating a report, you can quickly open Auto-Correct by selecting the text and clicking Speech > Auto-Correct. This option is only available when text is highlighted.

 

Misrecognition of a number when dictating a numbered list

When dictating a numbered list, at some point, usually on number four or five, the system will not recognize a number as a numbered list item.

For example:

Dictate: "Number one period blah blah blah period number two period blah blah blah period number three period blah blah blah period number four period blah blah blah period"

Result:

1. Blah blah blah.
2. Blah blah blah.
3. Blah blah blah. Number four

WORKAROUND:

PowerScribe has a numbered list feature that allows a user to quickly dictate a numbered list by using special voice commands, "Start Numbering" and "Stop Numbering". Using these commands enables the user to dictate content without the need to dictate the number for each list item. Here's an example of using the numbered list feature to dictate the Impression section of a report.

Dictate:
"Impression colon new paragraph"
"Start Numbering blah blah blah new line"
"blah blah blah new line"
"blah blah blah new line"
"Stop numbering"

Result:
IMPRESSION:

1. Blah blah blah
2. Blah blah blah
3. Blah blah blah

NOTE: When dictating "new line" the system executes a soft return (doesn't skip a line) and the next number is inserted automatically. Dictating "paragraph" or "new paragraph" will execute a normal return (skip one line) and the next number is inserted automatically increment.

 

Misrecognition of vertebrae when using "through"

Dictating "L two through three" results into "L2 through 3", or "L2 through three", instead of "L2-L3" or "L2-3".

If you dictate without a pause and include "L" for both numbers, the result is correct, however, if you pause slightly after the first "L" you will get "L2 through three", "Two through three", "L2 though L3", or "L 2-3".

WORKAROUND:

Dictating by using the word "to" as opposed to "through" provides better results, unless a lengthy pause occurs after the initial "L". The result, in almost all cases, will be "L2-3" or "L2-L3" depending upon how your custom formatting is configured.

 

Misrecognition of negative numbers when dictated in an Auto-Text field

A negative number will not populate correctly when dictated into an auto-text field that is formatted as a number. This only occurs when dictating the word "negative" prior to the number.

For example:

Dictate: "negative 1.5"

Result: "negative 1.5" instead of "-1.5."

Dictating by using the word "minus" as opposed to "negative" provides better results.

 

Misrecognition of Dates

The misrecognition occurs most often when the month and year portions are dictated with single digits. For example:

Dictate: "three zero seven zero six comma three zero eight zero six comma three zero nine zero six comma three one zero zero six comma three one one zero six comma three one two zero six comma three one three zero six comma three two one zero six comma three two two zero six comma zero three two two zero six

Result: 03/07/2006, 03/08/2006, 3/09/2006, 31006, 31106, 31206, 031306, 32106, 32206, 032206

Note: if you dictate the above DD as eleven, twelve, thirteen, twenty one, twenty two, the date is accurately recognized and formatted properly.

Note: This is not actually limited to dates prior to 2010 simply try using the year 13 and you will see the same result.

Dictate: zero three one three one three comma three two seven one three
Result: 031313, 32713

Note: if you dictate the MM (as thirteen and twenty seven) but use single digits for the YY the date will not work correctly.

Dictate: three thirteen one three comma three twenty seven one three
Result: 313 13, 320 713

Note: If you dictate the following, the date is correctly formatted every time.
Dictate: three thirteen thirteen, three twenty seven thirteen.
Result: 03/13/2013, 03/27/2013

WORKAROUND:

Dictating the date by following the recommendations listed below may help improve the recognition accuracy.

Day and Month:

  • For single digits (1-9): Dictate zero followed by the single digit. (I.e. "zero one" or "zero two")
  • For double digits (10-31): Dictate "ten", "eleven", "twelve" "thirteen", "fourteen", "twenty", "twenty one", "twenty two" etc.

Year:

  • For the 1900s: Dictate "nineteen" followed by the recommendations for single digits and double digits described for the day and month above.
  • For the 2000s:
    • For the year 2000: Dictate "two thousand" .
    • For years 2001 - 2009: Dictate "two thousand " followed by a single digit (I.e. "two thousand one", "two thousand 7", etc.)
    • For years 2010 - 2015: two thousand" followed by "ten", "eleven", "fifteen", etc. for two digit years.

NOTE: For some, dictating the month by name provides better recognition results. (I.e. "March fourth two thousand twelve" results into "03/04/2012")

 

Misrecognition of Height

When dictating "height sixty five point zero inches", the recognized text appears as "height 60 feet 5 inches and 0.0 inches".

As an alternative, consider dictating the height by saying, "Height colon". For example:

Dictate: "Height colon sixty five point zero inches period"

Result: "Height: 65.0 inches."

 

Misrecognition of words which are followed by a word beginning with an "S" sound

When dictating a word that is followed by a word beginning with an "S" sound, the initial word is incorrectly made plural.

For example:
a. Dictating "appear stable" results into "appears stable"
b. Dictating "bulge is" results into "bulges"
c. Dictating "change is" results into "changes"

WORKAROUND:

Training both words, in most cases, a will solve the recognition error. Perform the following steps to train each word:

  1. Launch the PowerScribe 360 | Reporting client application and Login.
  2. Say Edit Vocabulary or click Speech > Vocabulary Editor to open the Vocabulary Editor.
  3. Type cardiomediastinal into the Written Form text box.
  4. In the result list, click the word cardiomediastinal to highlight it.
  5. Click the Train button to open the Train Words dialog.
  6. Click the Go button and say, cardiomediastinal. The word will disappear after the system captures your voice.
  7. Click the Go button a second time and say, cardiomediastinal. the grey button will show green for a moment and the word will disappear again.
  8. Click the Done button to save the change and return to the Vocabulary Editor dialog.
  9. Repeat steps 3 to 8 using the word Cardiomegaly.
  10. Click the Close button to exit the Vocabulary Editor.

Voice Commands / Formatting - KNOWN ISSUES

Dictating "series and image numbers" mid sentence will start a numbered list.

When dictating, "series three image number one reveals a node", the system will execute a soft return for the word "number" followed by the beginning of a numbered list. For example:

Dictate: "series three image number one reveals a node"

Result:

1. Reveals a node

WORKAROUND:

There is a Speech Formatting option found in the PS360|R Admin Portal and the PS360|R Client. This speech formatting option, when enabled, allows for the system to automatically place a paragraph return before starting a numbered list. Disabling this option will help prevent the dictated sentence, which includes a numeral, to execute a paragraph return when a numeral is dictated. This is a temporary solution that helps in that the sentence will not be broken up by a paragraph return when dictating a numeral.

The steps below demonstrate how to disable a speech formatting option in the PS360|R Client to be:

  1. Launch the PowerScribe 360 | Reporting Client application and login with your credentials.
  2. On the Menu bar, click Speech > Formatting.
  3. Click the Numbers, Units and Dates tab.
  4. Use the drop-down box labeled Before a numbered list item, automatically insert: to select the Nothing option.
  5. Click Apply to save your changes.
  6. Click OK to exit Speech Formatting settings.

 

Formatting of "grams per square centimeter"

When dictating "grams per square inch" the system will format the text to read "g/sq cm". Although this statement is correct, some users prefer it to read "g/cm2"

Improper capitalization of the following word when dictating "ounce (oz.)"

When dictating "A five pound three ounce tumor was removed", it is recognized as "A 5 lbs. 3 oz. Tumor was removed", with tumor being incorrectly capitalized.

There are two settings in the PS360|R client that effect this outcome.

  1. Speech>Formatting>Abbreviate common English units of measure.
  2. Tools>Preferences>AutoCorrect>Capitalize the beginning of dictated sentences, however this setting causes issues with their AutoCorrects not being capitalized at the beginning of sentences.
