In this edition of eTIPS
you will find tips about how
providers can improve recognition
accuracy, how Microsoft® Word (MS Word)
2007 default line and paragraph
spacing impacts Dictaphone® Enterprise
Speech System (ESS) Worktype
templates, how sorting pools by
"Target Time" helps management meet target
turnaround time goals, and learn about what the
various Enterprise Turbo Speech processes do and
how often these processes run.

eTIPS
Did you know that Providers can take
steps to improve Enterprise Turbo Speech
Recognition
Accuracy?
There are
certain dictation techniques which can enhance the
recognition accuracy of Enterprise Turbo Speech
jobs. Keep in mind that even though speech
recognition technology continues to improve,
recognition accuracy is not typically 100%
perfect. However, by following certain
guidelines, Providers can have better
recognition by following the below
suggestions:
Dictation
Friendly Environment
-
Try to dictate in a quiet
area. Background noises such as
conversations or tones/noises from medical
equipment can cause recognition errors.
-
If there is some background
noise, try to keep the microphone facing away
from the source of the
noise.
Organize Thoughts Before
Dictating
-
Try to compose the sentence in
your head before dictating.
-
Dictate instructions to the
Transcriptionist at the end of the
dictation.
-
Try not to repeat or correct
phrases during the
dictation.
Proper Enunciation of
Words, etc.
-
Speak clearly - you may speak
quickly as long as the words are spoken
clearly.
-
Speak at a normal volume and
maintain that same volume throughout the
dictation. Fluctuations in volume
(especially a sudden decrease) may cause
recognition errors.
-
Keep the microphone at a
consistent distance from your mouth.
-
Try not to clear your throat,
yawn, chew gum, eat, or drink during a
dictation. This can cause insertions of
<NotRecognized> or recognition
errors.
-
Though not required, if
punctuation is spoken, it will increase the
accuracy of punctuation in the Draft text. For
example; by dictating "colon" after heading
words, this will ensure a particular word is
formatted as a Heading in the draft
text.

Did you know that
Microsoft changed the line and paragraph
spacing defaults in Word 2007, and this
change impacts Dictaphone ESS Text
template creation?
When Microsoft released Word 2007,
the default line and paragraph spacing was changed
from previous versions as shown
here:
When using MS Word 2007 to create
templates in the Dictaphone ESS Text
Explorer application, these new default
settings cause information in documents to
be different. To preclude this from
happening, it is essential that you change the
default settings in any Word 2007 environment
where Dictaphone ESS Text templates are
created.
To revert to MS Word 2003 default
line and paragraph spacing, do the
following:
-
Ensure that EX
Editor and Enterprise Speech System
Text Explorer applications are
NOT running on the
machine where you are making these
changes.
-
Open Word
2007.
-
With Home menu item selected,
choose Change Styles.
-
Select Style Set and click Word
2003.
-
Click Change Styles once
again.
-
Click Set as Default.
-
Exit Word
2007.
Dictaphone ESS Text templates
subsequently created in Word 2007 will now display
correctly.

Did you know sorting a pool by Target Time
offers management an efficient process to help
meet Target Turnaround Time goals?
The purpose of target time is to
appropriately prioritize the transcription of
various types of reports, each of which has a
different goal for turnaround time. Target time is
a method of balancing these potentially
conflicting goals.
This function allows hospitals,
clinics and transcription services to manage
turnaround goals, even if the service must balance
work from multiple facilities with different goals
for each type of report.
Example of Target
Time
A supervisor has a team of transcriptionists
who transcribe both History and Physical (H&P)
and Op Reports. The supervisor's goal is to have
targeted completion for Op Reports within 6-hours
and H&P reports within 24-hours from the date
and time dictated. The following shows the order
in which jobs are sent for transcription when
sorted by First-in-first-out (traditional sort
order) and by Target Time order. Notice the order
in which the transcriptionists are sent reports
changes significantly when sorted by Target Time.
This ensures the reports are sent for
transcription in an order more conducive to the
target turnaround time goal.
To create a workflow which supports Target
Time distribution, there are four areas that need
to be configured in the Dictaphone ESS
Voice Explorer
application including:

Create Target Time Definitions
for each type of dictation which is sent to the
Voice system. This is configured in the
Job Filter Definitions. To
accommodate the example above:
-
Create on Job Filter definition
with a criteria of WT 1 meeting a target
turnaround time of 24 hours.
-
Create a second Job Filter
definition with a criteria of WT 2 meeting a
target turnaround time of 6
hours.
- Create one pool which includes
criteria for all work the transcriptionist or
team will transcribe and select
Target Time as the sort option.
To accommodate the example above; create a pool
which includes criteria for WT 1 and 2, and sort
by Target Time.
- Create a Pool Script and assign this pool to
the pool script.
- Assign the Pool Script to the
transcriptionist(s).
Please Note: It is important
that only one target time pool be assigned to the
pool script. The one pool needs to include every
type of work the transcriptionist or team
will transcribe, which allows for all work to
be distributed in an order based on target
time. Assigning multiple target time
pools defeats the purpose. However, the
Target Time Pool can still be used in conjunction
with a Special Assigned or a Priority Pool, so
high priority jobs can always be moved ahead of
the others.
There are two Job Lister
columns that aid in monitoring Target Time
jobs:
-
Target Time
which displays the timestamp that was assigned
based on the Job Filter Definition
configurations. TIP: Sorting
the report output by this column in ascending
order will provide you with the order in which
the jobs will be distributed to the
transcriptionists.

Amount of Time
which displays how much time is left before the
job reaches it's designated Target
Time.

Guide related to your current version of ESS
software and navigate to the " Introduction
to Job Filters" section. If you are not

Did you know what the various
Enterprise Turbo Speech processes do and how often
they run? LMID
(Language Model Identification) - This
process analyzes the signed off documents and
compares the words within those files to the words
in each of the Language Models. The Language
Model with the most matches is selected for the
Provider. LMID runs weekly for each
Dictator.
Adaptation (aka Acoustic
Adaptation) - Each Provider has a
custom acoustic model file for each input device
they use. This represents the Provider's
speaking style and pronunciation of
words. These files are created when
Adaptation runs. Adaptation runs twice when
users are first setup for Enterprise Turbo
Speech.
Fluency Meter - Fluency
Meter runs for each Provider configured for
Enterprise Turbo Speech. It compares the
recognized documents to manually typed documents
to determine the dictation/recognition
quality. If the Provider qualifies for
Enterprise Turbo Speech, he/she is determined to
be Draft. If the Provider is not
suitable for Enterprise Turbo Speech, he/she is
determined to be Non-Speech. Fluency Meter
runs when users are newly added as Enterprise
Turbo Speech users.
LMA (Language Model
Adaptation) - This process identifies
words in the edited/signed-off documents that do
not exist in the Language Models. Those words
are then presented in the LM Adaptation tab of the
Customization Client. The words can then be
added to word lists for inclusion in the Language
Models or rejected so they don't show upon the LM
Adaptation list. Currently, LMA runs every 24
hours for each Provider. However, beginning
in version 8.0 Service Pack 1, this will
change to weekly.
TMA (Transformation Model
Adaptation) - This process, also known as
the Smart ReWrite™ feature, runs for
every Provider and it compares the
edited/signed-off documents to the unformatted
speech recognized text. It learns certain
corrections/modifications made by the
Transcriptionists and applies those changes to the
draft text for subsequent Enterprise Turbo Speech
jobs for that provider. Currently, TMA runs
every 24 hours for each Provider. However,
beginning in version 8.0 Service Pack 1, it
will be changed to weekly.

All
Aboard the Speech Express
Keys to Success:
Transcriptionists are saving time and increasing
productivity with speech editing. One big time
saver is to stop using the mouse and keep fingers
on the keyboard. Both Correction Client and EX
Editor are designed with this in mind; incorporate
keystrokes that move, remove, and add
content.
Magic Punctuation: By using
punctuation keys in combination with the
Control or Alt
keys, transcriptionists can quickly add periods,
commas, colons, semicolons, or start a new
paragraph. Unlike traditional transcription, the
transcriptionist does not have to be at the end of
the word to perform a magic
punctuation command. As long as the cursor is
touching the word or the word is highlighted, the
system will place the punctuation appropriately.
Did you know that Magic Punctuation will also
remove punctuation? These commands act as a
toggle; for example, if there is a period
that needs to be removed, invoking the Magic
Period will remove the period and change
the first letter of the following word to
lower case, "like magic."
Microsoft® Commands: Similar
to Nuance's special keys, time can also be saved
by using Microsoft commands within a document,
such as:
- Control+C to copy.
- Control+X to delete.
- Control+V to paste.
- Control+Z to undo.
- Shift+F3 to toggle through
capitalization options (ALL CAPS, Title Case,
and lower case).
- End to move to the end of a
line.
- Home to move to the
beginning of a line.
- Control+Up Arrow to move to
the beginning of a paragraph.
- Control+Down Arrow to move
to the end of a paragraph.
Introductory and Closing
Text: When draft text is populated in a
report it is positioned between two bookmarks
including "Start Typing" and "End Typing." When a
provider dictates introductory text such as
patient name, medical record number, etc., this
information is usually dictated first and can be
found just after the "Start Typing" bookmark.
Alternatively, when a provider dictates closing
text such as thank you, end of dictation, or
please carbon copy Dr...., this information is
usually dictated last and can be found just prior
to the "End Typing" bookmark. The following will
guide you through the steps to quickly remove both
Introductory and Closing Text:
Remove Introductory Text
- Place your cursor on the last
introductory word which needs to be removed.
- Press Shift+F11 to
highlight all the text included from the cursor
position to the "Start Typing" bookmark.
- Remove the highlighted text by
pressing the Delete
key.
Remove Closing Text
- Place the cursor on the first
word of the closing text which needs
to be removed.
- Press Alt+Shift+F11 to
highlight from the cursor position to the "End
Typing" bookmark.
- Remove the highlighted text by pressing the
Delete key.
TIP: Instead of pressing the
Delete key, you can keep your hands in the home
position and press Ctrl+D (EX Editor) or
Alt+D (Correction Client).
These are only a few of the commands
available, all of which are valuable tools in
increasing productivity. To learn more about

What's New
Dictaphone Enterprise Speech System
(ESS) 8.0 Service Pack 1
Service Pack 1 for the Dictaphone ESS is
scheduled for release in January of
2010. Some of the major highlights
of this Service Pack release are as follows:
Dictaphone Enterprise Speech System -
VoiceSystem Software
- In Voice Explorer, under Users, EXS Tab, the
Apply to Profile button was not working properly
for users without a license. It will now
work properly and apply the changes to anyone
that has a device enabled and that shares a
profile with the current user.
- Changes to the way QA Reasons are displayed
in the Voice Explorer Pool's Integrated QA
tab. The QA Reason Code now will have
the Text site that it belongs too
displayed. The displayed text will
be: EXTSite ReviewCode Description.
- In Voice Explorer, corrects an issue where
clicking under Pools | Users would sometimes
give a client side popup error.
- In Job Lister, corrected an issue
with displaying the date and time when
the client is in a different timezone.
Dictaphone Enterprise Speech System -
Enterprise Turbo Speech - Customization
Client
- Allow sorting of the list controls by
clicking on the column headers.
- Words will not be added to the LM Adapt
list if they're already anywhere in the other
tabs.
- Corrected an issue where the EXV server
would leave LST files locked if someone used the
customization client, and that client shut down
in a non-orderly fashion. The next time
someone ran the customization client, they would
receive errors about failing to download LST
files.
Dictaphone Enterprise Speech System -
Enterprise Workstation
- Implementation of the EWS functionality to
only download profile updates from the PSSDK on
a configurable time basis (i.e. once a day,
etc.) This has been proven
to shorten login time for sites with lower
bandwidths.
- Corrected an issue with users hitting the
ESC and RETURN keys while using EWS PACs Bridge.
- Fixes an issue that occurred
when a user signed off a job that contained
a shortcut with default field values. On
sign off, it would strip extra text from the
document. Used a different call to the
PSSDK to prevent this.
- Will now check if anomaly text is blank, and
also no longer allow sign off on a report
containing anomalies.
Dictaphone Enterprise Speech System -
TextSystem Software
- Split job now always starts at
split position in Word Client with headers
turned on.
- Provided additional configuration for
splitting a voice job, as follows:
- Confirmation for user splitting a voice job.
- Display of duplicate documents window for
user when starting a split job.
- A split job will remain as the top job unit
in the current session until the job is
finished, ensuring that higher priority jobs do
not interfere with completion of the
job.
- Users are no longer able to use Alt-C to
reject jobs unless a specific privilege has been
assigned to that user.
- In EX Editor, a option has been added to
sort the Visit list control by Date of Service.
- Correction Client 1.6.1.2, issued as a part
of the Version 8.0 Service Pack 1, now allows
the case of last letter of a token to be
changed following a magic period.

Correction Client Version
1.6.1.12 (for Enterprise Versions 7.5 and
below):
A new correction client was recently released
and includes the following enhancements:
- The Spell Check Macro now displays correctly
in both Draft and NBS Draft modes.
- Images included in work type template
headers will now display when transcriptionist
types in NBS mode.
- Magic period, as related to changing of
letter case, now works as
intended.
